Registrants > Registrant Search Page

The Registrants > Registrant Search page allows users to search for Registrants using specific search criteria.

To search for a Registrant, perform the following steps:

1. Select from the following search criteria:

   Search Criteria for Registrant Search Page

   User Information	Description
   Last Name	Enter the Registrant's last name.
   Last Four digits of SSN	To filter Registrant records by SSN, enter the last four digits of the Registrant's Social Security Number (SSN) in the provided box.
   Date of Birth	To filter the search results by the Registrant's date of birth, enter the date using the format mm/dd/yyyy.
   Registry Status	To filter the search results based on the Registrant's status, select one of the options from the provided dropdown.
   Registry Flags	To filter the search results to return only those Registrants that have had Administrative or Subpopulation Flags placed in their record, choose one of the available selections.
   Registrant ID	To filter the search results by the Registrant's unique ID number, enter that number in this box.

2. Click the Search button.

   The system displays a list of Registrants that meet the selected search criteria.

   

3. Click the Select button to view the Registrant's record.

   The Change Status > Registrant: LastName, FirstName > Clinically Relevant Summary page displays.

   

   Note: -_The LastName and FirstName variables correspond to the user's first and last names that are recorded in their account information.
